ANNUAL MEMBERSHIP MEETING PROXY

The undersigned, a voting member of Congregation Shir Ha-Ma'alot, a California nonprofit corporation, revoke any previous proxies and hereby appoint the then-serving secretary and treasurer of Congregation Shir Ha-Ma'alot, or either of them acting independently if only one of them is then available, as my proxy to attend the meeting of Congregation Shir Ha-Ma'alot's members on May 22, 2018 at 7:00 pm, and any adjournment or postponement of that meeting, and to vote or otherwise represent each membership which I am entitled to vote as to any matters other than election of Officers and Trustees.

I expressly intend that this proxy permit the person(s) identified above to attend the May 22, 2018 meeting on my behalf for purposes of establishing a quorum.
